The Nigerian Safety Investigation Bureau (NSIB) today released 20 safety recommendations on aviation and maritime accidents, including four immediate recommendations for Dana Air 5N-BKI.
These safety recommendations emerged from two aviation incident final reports, one aviation incident preliminary report, and a marine accident preliminary report.
Commitment to Safety
In his opening speech, NSIB Director General, Captain Alex Badeh Jr., stated the investigation team worked tirelessly to uncover the root causes of various incidents and accidents. They achieved this through meticulous analysis, thorough research, and unwavering dedication. Captain Badeh said, “The publication of the reports serves as a testament to the NSIB’s commitment to safety and underscores its mission to prevent similar occurrences in the future.”
Dana Air’s Immediate Recommendations
Director of Transport Investigation, Engr. Abdullahi Babanya, announced immediate safety recommendations for Dana Air. Three recommendations were directed to the Nigeria Civil Aviation Authority (NCAA) and one to the Nigeria Airspace Management Agency (NAMA).
The NSIB recommended NCAA should ensure Dana Air uses the Quick Reference Handbook timely during critical flight phases when dealing with abnormal and emergency situations. It advocated following standard operating procedures (SOPs). It suggested NCAA should also ensure Dana adheres to standard phraseology when communicating indication malfunctions during abnormal or emergency operations. The Investigators charged NCAA to review Dana Air’s training program and Operating Procedures to emphasize Crew Resource Management (CRM) and Threat and Error Management (TEM).
For NAMA, the recommendation was to ensure Air Traffic Controllers insist on communication of specific malfunctions using standard phraseology. For example, using “indicated system malfunction…” rather than “Gear Disagree.”
Maritime Safety Measures
NSIB also issued five safety recommendations in the preliminary report on the boat accident that occurred one nautical mile from Onono jetty in Anambra State en route to Asaba jetty in Delta State.
The Bureau proposed National Inland Waterways Authority should ensure all boats involved in commercial activities are registered. It recommended NIWA should enforce adequate safety measures in the inland waterways.
The NSIB advised that boat operators should receive increased sensitization on safe operations and security along the Anam river. It said training should be provided to local fishermen and boat association members as rescue divers for emergency rescues.
Furthermore, it advocated increased regulatory oversight to ensure adequate insurance for boats, drivers, and passengers operating on inland waterways.
Additional Reports
Moreover, the reports also include two final reports on other incidents. A ground collision occurred between a Lavatory Service Truck operated by SAHCO and a parked Max Air Boeing 737-300 aircraft. The final report details a serious incident involving an Air Peace Embraer EMB-145LR at Kaduna Airport on January 14, 2021.
